Steps, hazards and controls
Watch the task being done and write the steps in the order they actually happen. Then record the hazards of each step and the control that manages each hazard.

Involve the workers
The Health and Safety at Work Act 2015 requires worker engagement on health and safety matters. For a task analysis, that looks like this.
- The workers who do this task helped write the steps
- Steps describe what actually happens, not what the manual says should happen
- New or young workers walked through the task with a supervisor before doing it alone
- Workers know they can stop work if something is not right
- The completed analysis is kept where the crew can see it
- Changes suggested by workers were added and dated
- The analysis was talked through at a toolbox talk
- Workers signed the analysis before starting the task
Check your controls
Before settling on a control, test it against the hierarchy. PPE is the last layer, not the first.
- Can the hazardous step be removed from the task entirely
- Can a less hazardous method, tool or substance do the same job
- Can the hazard be isolated with guarding, barriers or lockout
- Do engineering controls reduce the risk at the source
- Are administrative controls (rotation, procedure, signage) written down, not just understood
- Is PPE specified by type, not just 'wear PPE'
- Does any control introduce a new hazard of its own
- Is someone named to check the controls are actually being used
Sign-off and review
Sign before starting. Revisit the analysis when the task, tools or people change, and after any incident or near miss involving the task.
